A Study of measurement of the interval between the anterior, arch of the atlas and the odontoid process


Abstract
The diagnosis of subluxation or dislocation in atlanto-axial joint is difficult with ordinary method. Purpose of this investigation was to measure the interval between the anterior arch of the atlas and the odontoid process for detection of laxity or injury to transverse ligament in cases of trauma, rheumatoid arthritis, degenerative arthritis, congenital anomaly and others.
The author has analysed and measured 21 cases who were admitted or visited with pain on neck to department of orthopedic surgery, Korea University for 6 months from April 1972. to Sept 1972. The results of clinical analysis and measurement are followings.
1) 11 cases out of 21 cases were male and 10 cases were female. The distribution of age was wide from 5 months of age to 67 years of age and frequent in 41~50 years of age group.
2) 7 cases(42.8%) out of them had the trauma on neck as the causative agent, 6 cases with rheumatoid arthrits, 4 cases with degenerative arthritis, 2 cases with congenital anomaly.
3) Duration of symtoms has wide variety and 7 cases of them (33.4%) had within 1 week.
4) The measurement and it¢¥s result about the interval between postererior aspect of anterior arch of atlas and anterior margin of odontoid process of axis on lateral projection were following. In neutral position, 2.25mm. on male, 1.99mm. in female. In full flexion, 2.8mm. in male, 2.45mm. in female. In full extension, 2.24mm. in male, 1.93mm. in female.
5) It is very important to measure the interval and check the laxity of transverse ligament with lateral projection in full flexion and full extension status.
